These instructions apply to both Oahu and Neighbor Island bookings:

1. Click on ‘Download Bill of Lading Form‘ button below.
2. Download the PDF form to your computer.
3. Fill out the form and save the completed version to your computer. Rename the file to include your name followed by bol-form.pdf.
4. Email us your completed form by sending it to distribution@pacifictransfer.com
